Joseph’s Home and St. Vincent Charity Medical Center partner to treat diabetes

Mr. Craft turned to St. Vincent Charity Medical Center to treat complications related to diabetes. He visited the Emergency Department 38 times in one year, including 13 times in the span of just two months. Over the course of that year, Mr. Craft had been living with friends. With no home to call his own, he was not getting the nutrition he needed and did not have the ability to keep his diabetes under control. So when Mr. Craft visited St. Vincent for the last time, the caring ED staff would not release him until he had a safe environment to stabilize his diabetes. Social workers at St. Vincent referred Mr. Craft to Joseph’s Home, temporary housing that provides medical respite for acute illnesses such as diabetes.

As part of his release from the ED and move to Joseph’s Home, Mr. Craft was also referred to the Diabetes Education Program provided by St. Vincent, a month-long, four-course self-managed plan that covers blood sugar management, meal planning, benefits of exercise, diabetes medicine, health problems associated with diabetes, and how to live well with the disease.

With the help of Joseph’s Home and St. Vincent Charity staff, Mr. Craft has since learned how to administer insulin and check his blood sugar levels. He has also been attending regular support group meetings, all at no cost to him. The collaboration between these two ministries of the Sisters of Charity Health System and the support of friends like you has enabled Mr. Craft to receive the care he needs to manage his diabetes, strive to live a healthy lifestyle and find permanent housing.
